Psychiatric Evaluation Intake
Service Description
A comprehensive psychiatric evaluation for adults experiencing concerns such as anxiety, depression, ADHD, insomnia, mood changes, or other mental health symptoms. Your visit includes a detailed review of your current symptoms, psychiatric and medical history, medications, lifestyle factors, and treatment goals. An individualized treatment plan will be developed, and medication may be prescribed when clinically appropriate.
Cancellation Policy
Luxe Med Spa is a self-pay practice and does not bill or accept health insurance for Health & Wellness or Psychiatry services. Payment is due at the time of service. We accept eligible FSA/HSA and flexible spending cards when applicable. Upon request, an itemized receipt can be provided for you to submit to your insurance company or benefits administrator for possible out-of-network reimbursement. Reimbursement is not guaranteed and is determined by your individual insurance plan or benefits administrator. Patients are responsible for verifying coverage, eligibility, and reimbursement requirements directly with their insurance or FSA/HSA provider.
